The Intake Coordinator for our Clinical Treatment program would be the first point of contact with applicants and referral sources. This position is tasked with specific responsibilities that meet contractual requirements and program goals while maintaining high standards of care. This includes supervising the intake process, ensuring all components of the intake process have been completed and successfully transfers the new resident to an assigned case manager. Additionally, the Intake Coordinator conducts outreach to various community-based organizations to discuss referrals, attends appropriate community-based and program-based meetings.

Responsibilities

Meet with prospective clients to provide applications, conduct initial assessments, formulate a recommendation to the VRC Program Director, and make referrals to other agencies, as needed.

Meet with prospective clients and conduct initial and ongoing assessment of clients’ appropriateness to the program.

Ensure admission eligibility by conducting the following:

  1. -Review application with Mental Health Assessor and the Nurse;
  2. -Check for outstanding warrants;
  3. -Verify funding source

Maintain demographic and other needed information regarding all clients seeking assistance

Formulate a recommendation for either acceptance to the program or reasons to refer the client elsewhere.

Attend weekly intake review meetings; staff prospective clients as needed to ensure clients are admitted in a timely fashion.

Communicate with prospective clients and referral sources on the status of an application.

Establish and maintain both an intake schedule and if needed, a waiting list for prospective clients

Coordinate treatment with the assigned case manager, and conduct an initial tour with the client to introduce her/him to the program and campus.

Create resident badge

Photocopy necessary identification documentation to include photo id and DD214

Perform crisis intervention and other counseling duties as needed.

Refer clients as needed.

Assure that each applicant is provided appropriate services or referrals and utilizes program components that are consistent with the client’s treatment needs.

•  Utilize knowledge of counseling modalities to match the appropriate treatment to client issues.

•  Assess clients’ appropriateness on therapeutic issues throughout the intake process.

•  Refer client to appropriate (VVSD and outside resource) specialized issues groups.

Refer clients, as appropriate to their individual needs, to outside resources. (e.g. medical, dental, mental health, housing, legal, social services, veterans benefits, etc.)

•  Interface with other agency staff as appropriate regarding client needs and potential for treatment at the VRC.

Develop and deliver case presentations at staff meetings to introduce applications of prospective clients.

Develop a collegial relationship with referral sources and work as a team on client interventions when appropriate.

Respond to queries from prospective clients and referral sources in a timely fashion.

Conduct assessments at other sites, when approved.

•  Review applications that have been submitted by approved referral sources (i.e. Public Defender, VA Liaison).

•  Oversee the Referral and Intake processes.

 

•  Ensure all intake supplies are readily available, (e.g. intake packets, binders, ID badges, etc.)

•  Utilize staff and model residents as appropriate.

•  Maintain HIPAA guidelines with regards to applications and client files.

•  Submit a Daily Status Report and Funding and Clients for Case Managers report which accurately identifies:

  1. -the total number of clients in the program per funding source;
  2. -breakdown of clients per funding sources (e.g. East, Upper East and West);
  3. -breakdown of clients per case manager;
  4. -including program admissions and discharges and admissions/discharges to/from the

hospital.

•  Manage the Food Stamp application process and prepare clients to meet with the Program Administrator at the Food Stamps office.

  1. Fax applications to the Health and Human Services;
  2. Coordinate appointments and set up transportation to/from appointments.

 

•  Handle and safeguard medications according to VRC nursing guidelines.

•  Monitor the Norchem billing report and match clients/funding with invoices.

•  Collect and handle urinalysis specimens for controlled substance testing.

•  Complete documentation and maintain adequate urine specimen custody required for urinalysis specimens for controlled substance testing.

•  Attend staff meetings

•  Attend weekly case management meetings to receive information about current program issues, exchanges information with other staff regarding client needs, problems, and progress and to participate in staffing special applications.

•  Attend other clinical meetings as assigned by supervisor.

•  Actively participate in supervision meetings as required.

•  Other duties as assigned by supervisor

Qualifications

Must have current AOD CCAPP Certification

Minimum of an AA degree in Human Services or related field

Prepare daily status reports and monthly reports

Have at least 1 year paid experience in an adult residential treatment setting

Must be proficient in Microsoft Word and Excel and possess good data entry skills

Prior experience working with the homeless population preferred

Knowledge of community resources for veterans

Self motivated and able to work with little oversight

Team player

Able to work with diverse population

Basic understanding of veterans benefits and issues

Must be able to comfortably, both mentally and physically, perform all aspects of the job.

Must be able to sit for long periods

Valid California driver’s license, current vehicle insurance

Other duties as assigned by supervisor
