The Registered Nurse work with residents who suffer from chronic mental illness and substance abuse problems and know signs of crisis, possible relapse, and stages of motivation and recovery. Responsibilities
  • Demonstrates familiarity with 12-step approach or is willing to self-educate re: addiction as an illness, MAT, combat-related PTSD, and other topics related to substance abuse.   
  • Provides training to staff in handling medications 
  • Provide education to residents regarding medications, and chronic medical and mental health conditions.   
  • Notifies case managers, clinical staff, and residents about significant changes in residents’ medications or health.  
  • Provides education-related classes and groups including nutrition, medications, healthy lifestyles, and effects of poor health practices. 
  • Works with the VA, Family health Centers of San Diego and other public health and agencies, case managers, and others regarding residents’ health concerns as needed.  
  • Assists with medication storage and proper disposal of meds. 
  • Familiarity with chronic illnesses and infectious diseases such as COPD, heart disease, diabetes, SUD, mental health diagnoses, HIV, AIDS, HEPATITIS, and TB treatment and education. 
  • Assists contract physician with medical screening of new admissions 
  • Monitors, coordinates with case managers, schedules and documents that all new residents have documentation of Tb testing within agency’s required timeframe, as well as  COVID 19 for new residents and other residents upon request, for those who have had close-contact exposure with positive persons. 
  • Schedules all new residents for physical exam with contract physician if needed, within agency time frame, and maintains documentation 
  • Provides notification to case managers when clients do not show up for required Tb tests, COVID 19 tests and/or physical exams. 
  • Provides documentation of completed TB, COVID 19 and health exams to case managers for follow up and filing. 
  • Works with Family Health Center’s health care case manager for referrals for vision and dental care needs, SDI and Medi-Cal enrollment. 
  • In the absence of FHC case manager, provides referral assistance and advocacy for vision and dental needs to community health clinics or emergency rooms. 
  • Works with residents and Family Health Center staff for enrollment in Medi-Cal or other designated health insurance programs 
  • Maintains a roster of residents with limited activity, health restrictions and/or other modifications to accommodate disability, for chores and KP. 
  • Enters nursing case notes for incidents; significant physical and mental health changes, and/or critical medication concerns  
  • Provides bed passes and time out passes per policy 
  • Oversees the Pharmacy Technicians. 
  • Provides First Aid to residents and staff 
  • Completes Incident Reports as needed 
  • Provides In-service Training to staff as requested 
  • Coordinates with Family Health Center staff who provide education and HIV/Hep C testing on site 
  • Documents residents class attendance in Service Point 
  • Attends Manager Meetings, Safely Committee, Family Health Center, Process & Improvement meetings. Attends or represents VVSD at off-site meetings as requested. 
  • Monitors employee time clock use daily, and other supervisory duties—vacation approvals, performance reviews, interviewing, scheduling, task assignment, etc.  
  • Infection Control functions—determine whether Isolation is needed based on diagnosis or symptoms; report trends to management; provide education on prevention and self-care to residents and staff 
  • Inventory, request and stock supplies for residents’ health and hygiene, for cleaning and disinfecting nursing office and for the med room 
  • Review applications for admission to program when requested, for medical appropriateness, the need for detox and for reasonable accommodation for medical disabilities 
  • Interface with outside allied health providers to coordinate pre-op, post-op or after care when residents are hospitalized 
  • Request DME, equipment or special medical supplies when needed by residents and not in stock  
  • Provide written communication to dietary re: food allergies, texture modifications or other special diet accommodations to Kitchen staff 
  • Monitor and provide information to case managers and Program Director regarding residents’ use of prescribed Controlled Substances beyond what is prescribed or outlined in policy 
  • Provide orientation, guidance and feedback to staff who work in the med room 
  • Modify, recommend, update and revise Policies and Procedures relating to Medications, and general health and wellness and submit to P&I for discussion 
  • Other duties as assigned by Program Director 
Qualifications
  •  Current California Registered Nurse (RN) License
  • Must be able to fluently read, write and communicate in English 
  • Minimum 2 years paid experience as a Registered Nurse in an outpatient or residential setting, without direct oversight or supervision
  • Must be able to stand a minimum of 2 consecutive hours 
  • Must have strong computer skills, Excel, Word, Microsoft Office 
  • Must be able to work with diverse population 
  • Must be able to adhere to strong boundaries 
  • Must be detailed oriented and accurate 
  • Must have excellent record keeping skills 
  • Must be able to work with constant interruptions 
  • Must be able to, comfortably, perform all aspects of job description and other duties as assigned by supervisor 
  • Negative TB test annually; bi-annually if Chest X-Ray test is required 
  • CPR/First Aid training bi-annually. 

Physical Requirements:

• Ability to bend and lift light objects

• Ability to sit for extended periods of time while viewing a computer monitor

• Dexterity of hands and fingers to operate a computer keyboard

• Capable of hearing and speaking to exchange information

• Seeing to read reports and data

• Ability to speak English

• Must be able to work in a noisy, busy environment.
